EHR

The EHR in my mind is a medical record with computable data while EMR signifies free text. Structured documentation is the basis for the EHR which means the provider needs to learn a new language for documenting his findings and it varies from one system to the other. If the EHR is to be adopted it needs to be natural to the clinician and not a lot of data that is more or less useless. There is a way the have the structured data done by the computer using Ontology and Natural Language Processing:it is operational but not scaled so far. Leo E Cousineau MD
